Your company or institution should have an approved version of this form, which you should, of course.  You should be aware of what is on it and ensure that it and the process for documenting patentable ideas are known in the organization.  Here is a sample form in case your organization does not have one.

While the Invention Disclosure Form is important to capture ideas as they emerge, it is really just the trigger for a more in-depth discussion.  It can also be useful for all managers to understand the nature of such discussions, and the questionnaire can help get you there.

Role of each inventor
Collaborators (clarifying what they did in relation to the research / publication (to distinguish from "Invention"
Was the invention facilitated in the conduct of other work?
Were materials provided by others?
Were AI tool used, and how?
"I have discussed the invention with X prior to submission.
Consent from all inventors.   All inventors sign.
